Nashoba Nursing Service & Hospice, Shirley, MA, is pleased to announce Melissa Immonen, Community Liaison, to the agency. Melissa joins Kathleen Hubbard in the formation of our team to foster safe transitions from acute and long term care settings, physician offices, and homes to our agency for home health, hospice, and public health services. As part of the Nashoba Associated Boards of Health, NNS&H strives to provide exemplary care and services to our community members and Melissa brings great experience in the Hospice arena.
Melissa is a master level social worker with over 10 years of experience in home health and hospice. She received her B.A. in psychology from Suffolk University and her M.S.W. from Washington University in St. Louis. Melissa served as a medical social worker for 8 years prior to transitioning her skills into the Community Liaison position.
Melissa joins us from Arizona where she was working as a Community Liaison for the second largest hospice organization in the Phoenix market. Melissa was a leader in her market, earning recognition for increasing referrals from new sources. Melissa is also known for her community involvement, participating in several coalitions andorganizations, including serving on the Phoenix Area Agency on Aging’s Advisory Council.
Melissa is available to meet with patients, families and community members to discuss end of life choices, describe the Hospice insurance benefit and foster a patient centered approach. She will be serving the southern communities in our service area, while Kathleen will be focusing her efforts on the northern communities.
Kathleen Hubbard is the Business Development Manager. She is a graduate of Mount Wachusett Community College with her ADN and Fitchburg State University with her BSN and a minor in Psychology. She maintains her BSN, RN, and is also OASIS and ICD9 Coding certified. Kathleen has 23 years of experience in home health and hospice, IV management and acute care. Her extensive knowledge of the community and insurance requirements, as well as excellence in patient care makes her a leader at Nashoba Nursing Service & Hospice and in the community.
